Summary: Establishes policy and procedures to be followed in the performance appraisal of state and county classified personnel.

The Employee Performance Evaluation Form was developed by the State Office of Personnel Management, Department of Finance and Administration, to comply with legislative mandate.  The performance of all classified employees is evaluated using this instrument.
